Raising Hope for Families with Lyme

Applying for a Grant

Please complete the following steps to submit your online application. It is important that you read through all of the steps before starting the application process and review the Eligibility, Deadlines and FAQs. If you are applying for more than one child, please provide a separate application and Doctor’s letter for each child.

If you are selected for a grant, you are required to sign an online pledge form committing to provide feedback. You will also be required to submit receipts verifying use of funds and complete an anonymous online survey.

LymeLight Foundation awards grants to eligible children and young adults through age 25 at the time of application. It is the goal and the hope of the Foundation that through these grants children receive proper treatment and medication in their fight against Lyme disease. Grants are awarded in incremental amounts. The maximum lifetime grant is $10,000 per eligible individual.

Step 1: Application

For the online application, the parent or child/young adult will answer questions explaining the need for the grant and how the funds will be used. If you are applying for more than one child, please provide a separate application for each child.

Step 2: Medical Eligibility

LymeLight Foundation will determine medical eligibility with the help of the applicant’s treating physician. The letter must be on the doctor’s letterhead and addressed to LymeLight Foundation. The letter must confirm the diagnosis of Lyme disease and state the applicant’s name, date of birth and provide a brief case history. In order to maximize the potential to be selected, it is helpful to have the doctor speak to the financial need. A positive lab test does not replace the need for a doctor letter. A treating physician is defined as a Lyme-literate MD, ND, DO or NP. NDs must have graduated from a CNME accredited institution. The grant application is not complete and eligible for consideration by the Board of Directors until LymeLight receives the doctor’s letter.

Step 3: Support (Optional)

Feel free to submit additional letters of support from friends, family members, teachers, church, and/or additional members of your medical team, etc. These letters will be used to help us better understand your needs.

Letters may be submitted:
